What is Clomiphene?
Clomiphene citrate is an oral selective estrogen receptor modulator used for treating female infertility. By binding to estrogen receptors in the hypothalamus, clomiphene blocks estrogen’s negative feedback. This causes the increased release of two gonadotropin hormones: follicle-stimulating hormone (FSH) and luteinizing hormone (LH).
Clomiphene, also known as Clomifene, is a research chemical that is a nonsteroidal SERM, Selective Estrogen Receptor Modulator, which inhibits estrogen receptors located in the hypothalamus. It stimulates the hypothalamus to release GnRH in the body along with the anterior pituitary to release gonadotropin. Chemically it is a derivative of triphenylethelene and a mixture of zuclomifene ((Z)-clomifene) and enclomifene ((E)-clomifene) which are geometric isomers which are the reason clomiphene citrate has both estrogenic and antiestrogenic properties. Clomiphene has been used to treat infertility in women who don’t ovulate, male hypogonadism instead of testosterone replacement therapy and to treat Gynecomastia.

Research on Clomiphene successfully addressed infertility in women who do not ovulate, leading to its approval for use in the USA in 1967. However, there is no conclusive data supporting its efficacy for unexplained female infertility. Notably, the use of Clomiphene in test subjects increased the likelihood of giving birth to twins.
Additionally, studies have highlighted the positive effects of Clomiphene in treating hypogonadism in males as an alternative to testosterone replacement therapy. Research involving test subjects demonstrated a 2-2.5 times increase in testosterone levels among hypogonadal males.
Moreover, data indicates Clomiphene’s potential in assisting males with gynecomastia, a condition characterized by the accumulation of breast tissue due to an excess of estrogen in the body. This phenomenon may arise when males discontinue testosterone replacement therapy.
What is Clomiphene Used For?
Clomiphene is primarily used to stimulate ovulation in women with polycystic ovarian syndrome (PCOS) or other ovulation disorders. It aims to help women ovulate regularly and at a predictable time so that timed intercourse or insemination can be planned.
While not always successful, clomiphene improves the monthly chances of conception for many infertile women. It may be used alone or with other injectable fertility drugs.
What are the Benefits of Clomiphene?
For me, the main benefit has been finally ovulating predictably. My previous irregular cycles made it impossible to plan conception attempts properly.
After three clomiphene treatment cycles, I am now ovulating like clockwork on days 14 or 15 of my cycle. I feel more in control of my fertility. While not yet pregnant, regular ovulation significantly improves the odds.
Clomiphene also made my periods more regular with less painful cramping. I’ve noticed fewer mood swings and food cravings as well. My doctor says my hormone profile looks much better.
What Major Side Effects Does Clomiphene Have?
The main side effects I experienced were hot flashes, headaches, bloating, and breast tenderness. Occasional nausea and eye floaters occurred too. The hot flashes and headaches subsided over time but can flare up.
Of concern are potential risks like ovarian hyperstimulation syndrome, multiple births, and visual changes. So far, monitoring has not uncovered any serious issues, but it is critical to have close oversight when using fertility drugs.
Dosing Information of Clomiphene
The standard clomiphene dosage for ovulation induction is 50 mg taken orally for 5 days, beginning on cycle days 3–5. I took 100 mg per day for 5 days. Higher doses can be used if needed. Treatment is generally given for 3–6 cycles.
If pregnancy occurs, clomiphene should be immediately stopped. Always adhere to the dosing schedule recommended by your reproductive endocrinologist. I took my pills at the same time each morning.
